El Pinoy Care Services is looking for a full-time Care Coordinator to join our Head Office team in Campbelltown, supporting both NDIS and Aged Care clients.

A Day in the Life:
As a Care Coordinator, you will:
Manage a portfolio of NDIS and Aged Care clients.
Coordinate services to ensure clients receive high-quality, person-centred support.
Build strong relationships with clients, families, support workers and external stakeholders.
Conduct regular client welfare checks and maintain ongoing communication.
Assist with client onboarding, service reviews and care planning.
Work closely with the rostering team to ensure services are delivered effectively.
Maintain accurate client records, case notes and compliance documentation.
Escalate complex matters to the Care Manager / Team Leader where required.
Contribute to continuous improvements in client service delivery.
Selection Criteria
Qualifications & Experience
Previous experience in Care Coordination, Care Management, Case Management or a similar role within the NDIS, Aged Care or Community Services sector is highly regarded.
Certificate III or IV in Individual Support, Community Services, Disability, Ageing or a related qualification is desirable.
Current NDIS Worker Screening Check (or willingness to obtain).
Current National Police Check.
Current driver's licence and access to a reliable vehicle.
Proficient computer skills, including Microsoft Office and client management systems.

Strong problem-solving and relationship-building skills.

Native-level English communication skills are essential.
Filipino language skills are highly regarded due to the cultural diversity of our clients and workforce.
